Egypt climates is reasonable in Winter, Spring and Autumn, but very hot during Summer. So the best time to visit Egypt is during the northern hemisphere's Fall, Winter, or Spring (October to May), and it’s better to avoid June, July and August unless you will spend your Vacation at the Sea side in Alexandria, Northern Cost, Hurghada or Sharm Sheikh.
Yes, US citizens need a visa to enter Egypt but you don’t need to apply for iy prior arrival as you can issue it upon arrival at Egyptian airports for $25 for a single entry. A multiple entry visa is also available for $60.

If you mean the Pyramids area, yes like any archeological site, the area has entrance fees, which is usually included in the Tour Package you buy from the Tour Operator or the vacation organizer.
And if you mean Entrance inside the Pyramid itself, yes you will need an extra ticket to go inside. But it’s to note that it’s not recommended for people who had cardiac disease, hyper-blood tension or for pregnant women. Also claustrophobic persons are not recommended to go inside.

The best time to visit Egypt is starting from September till May. During Autumn, Winter and Spring the weather in Egypt is modest especially for the citizens of the North half of the Earth. During summer the weather is too hot to visit Upper Egypt, but it’s good time to be spend on the sea side.
Noting also that summer is a low touristic season in Egypt and Packages prices are also low.

There is no mandatory dress code for women in Egypt. But like most Middle Eastern countries, Ladies should be dressed conservatively. Cleavage, knees, and shoulders should all be covered. On all Egyptian beaches, swimsuits are permitted.
